Manager David Moyes had said the Dutchman was OK and had a chance to play after recovering from a groin problem, but he is not even on the bench as Manchester United make three changes from the side that drew 2-2 with Tottenham at the weekend.
In comes right-back Rafael, and midfielder Marouane Fellaini and Ryan Giggs, who all start, while Tom Cleverley, Chris Smalling and the suspended Phil Jones make way.
Fellaini faces his former side for the first time since leaving in the summer, so too is boss Moyes, who left Goodison Park for Old Trafford at the end of last season.
For Everton, Ross Barkley and Kevin Mirrallas both return to Roberto Martinez’s starting side, after they were on the bench for the 4-0 win against Stoke City at the weekend.
